- Still, his sleep might be considered deviated. Among the peculiarities of children's sleep, it is important to mention the statistic phenomenon. ... It is very helpful to have some information to be able to distinguish between deviations and normal sleep features. ...
- Old medical books make frequent reference to the strange way some people sleep. In recent years, we have had a chance to rediscover the importance of body positions in sleep, especially for treatment of Obstructive Sleep Apnea Syndrome in adults. Unusual and peculiar positions in sleep are common in children as well. ...

- Synopsis: This chapter reviews the literature on the effects of trauma and stress on children’s sleep patterns. ... The importance of distinguishing between subjective and objective measures of sleep as well as the two basic modes of response of the sleep-wake system are highlighted. ...
- Stress, Trauma and Sleep in Children.
- The relationship between stress and sleep is complex and multidimensional. To the extent that sleep and dreaming phenomena are sensitive to physiological and psychological stress, sleep, and sleep-related issues may arouse significant fears and anxieties in many children and thus represent a significant stressor in their lives. In addition, sleep disturbances and the resulting sleep loss or sleep deprivation and daytime fatigue and sleepiness can become a significant source of stress and require adaptational resources. The following review focuses only on the effects of stressful and traumatic experiences on sleep-wake patterns, summarizing the research and clinical literature in the field and outlining the theoretical and methodological issues that require additional research. ...
- Within Selye’s framework, atypical alternations in rest-activity cycles, hypervigilance, fatigue, and sleep-wake disorders represent non-specific components of the GAS.
- Sleep is very sensitive to transient as well as chronic aspects of the child’s emotional status, expectations, anxieties, and psychopathology. Sleep disruptions appear in many psychiatric disorders and in some of the DSM-IV disorders sleep-related problems and derivatives are among the diagnostic criteria. Despite their relevance, the present review will not address the relationships between sleep and psychopathology (see Dahl and Puig-Antich,15 for a review). ...
- Sleep in itself could be considered a stressor in many situations. Our ancestors believed sleep to be a death and the following awakening a rebirth. The interpretation of sleep as a final or total separation remains profound in human psychosocial organization throughout the life cycle. Going to sleep or falling asleep involves a series of processes that are potentially stressful, particularly to a young child. ... The fusion of cultural myths and developmental issues is clearly seen in the phase-specific normal fears of sleep and darkness and in the nightmares so prevalent in kindergarten- and school-aged children. ...

- Some Notes on Sleep Disturbances in Children & Adolescents.
- The majority of children and adolescents experience sleep problems at some stage. ...
- Sleep disorders are classified into two major categories; dyssomnias and parasomnias, and as such the following discussion of sleep difficulties will be presented within the context of these categories. Finally, a number of treatment approaches for sleep disturbances will be outlined.
- The Phenomenon of Sleep.
- The human sleep-wake cycle tends to display a longer period, of approximately 25 hours, when an individual is deprived of environmental time cues. ...
- Sleep occurs in a rhythmic pattern which is characterised by five distinct physiological stages, that are cycled through several times during the course of a nights sleep. Of these stages, the most important appear to be rapid eye movement (rem) and slow wave sleep (sws), as they are believed to support a number of important functions (Moore-Ede, 1982). ...
- While it does appear probable that sleep involves restorative and information storage processes, a comprehensive and unequivocal explanation to the question of why we sleep is still yet to be provided. Nevertheless, the debilitating effects of inadequate sleep are a sound indication that inadequate sleep, at the very least, has secondary effects that are not conducive to optimal functioning and wellbeing, and at worst, may have primary effects that impede physical and cognitive development.
- The Normal Developmental Sequence of Sleep.
- Although sleep is controlled by the same neural mechanisms for all people, the structure and pattern of sleep varies across different age groups. Children are born with a multiphasic sleep pattern that does not develop into an adult-like monophasic sleep pattern for a number of years. The emergence of a mature sleep-wake rhythm is a developmental process, which entails the child learning to adapt his or her sleep to comply with the 24-hour environment. Gradually the child’s sleep-wake pattern will consist of progressively shorter and fewer nap’s during the day, and a longer more sustained sleep episode at night. A monophasic pattern of sleep, synchronised to the environment, commonly emerges between the ages of 2 and 5, when the child completely abandons the daytime nap. The total amount of sleep required each 24-hour period, and the total amount of sleep time spent in rapid eye movement (rem) sleep, also follows a developmental time course, which steadily decreases with age, from approximately 16 hours per day in newborn babies, to approximately 8 hours per day in 17 year old adolescents (Horne, 1992; Edwards & Christopherson, 1994). Thus, the normal range of sleep duration in children varies enormously with age.

- Technical Report: Diagnosis and Management of Childhood Obstructive Sleep Apnea Syndrome .
- This clinical practice guideline, intended for use by primary care clinicians, provides recommendations for the diagnosis and management of obstructive sleep apnea syndrome (OSAS).
- The guideline contains the following recommendations for the diagnosis of OSAS: 1) all children should be screened for snoring; 2) complex high-risk patients should be referred to a specialist; 3) patients with cardiorespiratory failure cannot await elective evaluation; 4) diagnostic evaluation is useful in discriminating between primary snoring and OSAS, the gold standard being polysomnography; 5) adenotonsillectomy is the first line of treatment for most children, and continuous positive airway pressure is an option for those who are not candidates for surgery or do not respond to surgery; 6) high-risk patients should be monitored as inpatients postoperatively; 7) patients should be reevaluated postoperatively to determine whether additional treatment is required.
- This clinical practice guideline is not intended as a sole source of guidance in the evaluation of children with OSAS. ... It is not intended to replace clinical judgment or to establish a protocol for all children with this condition and may not provide the only appropriate approach to this problem. - Clinical services for sleep disorders .
- University Section of Child Psychiatry, Park Hospital for Children, Old Road, Headington, Oxford OX3 7LQ, UK.
- Children's sleep disorders are common and often harmful to development and well being. The clinical services available to affected children and their families need to be improved. At present, professional interest and expertise in sleep disorders medicine is severely limited by the paucity of appropriate teaching and training. The work of a mainly tertiary sleep disorders clinic was reviewed, which showed that accurate diagnosis of a wide range of sleep disorders is possible, and that treatment needs can be specified. ... A three tier system of service provision is proposed to improve this situation, which rests essentially on better professional training in the sleep disorders field.
- Keywords: sleep disorders; health services .
- Children's sleep disorders are common; they are diverse in type, aetiology, and treatment requirements, and frequently serious in their effects on various aspects of development. Ample justification for such assertions is contained in the two recent comprehensive accounts of paediatric sleep disorders medicine by Ferber and Kryger1 and Dahl,2 and in the reviews by Stores3 and Rosen. 4 The main points worth emphasising are: (1) ~ 25% of children, from infancy to adolescence, have sleep problems of major concern to them and their families; (2) the factors underlying such problems cover many physical and psychological possibilities; (3) the types of sleep disorder and varieties of treatment possibilities are much greater in number than conventional accounts in textbooks suggest; and (4) if these sleep problems are overlooked or mishandled, their effects on psychological and sometimes physical development can be serious and potentially long lasting. All these points apply to children in general; children in certain clinical groups, notably those with chronic physical illness, psychiatric disturbance, or learning disability, are at an even greater risk of sleep disorders and their consequences (Stores, unpublished data, 1998). ...
- In spite of the compelling argument that sleep and its disorders are a very important aspect of child development and paediatric practice, the topic features very little in the teaching and training of doctors and other professionals including paediatricians, psychologists, and others involved in the care of children. The median time formally allocated to UK medical student teaching in sleep and its disorders was recently shown to be five minutes overall: five minutes in the paediatric and psychiatric parts of the course, and zero minutes in the general practice course. ...
- Hard evidence about the provision of clinical services for children with sleep disorders and their families does not exist. ... In 1993, the British Sleep Society compiled a list of centres in the UK providing some type of sleep disorders clinic. ... Twenty four were essentially concerned with respiratory sleep disorders and only two of the total indicated a paediatric component. ...

- HAVING YOUR CHILD EVALUATED FOR OBSTRUCTIVE SLEEP APNEA .
- If you suspect that your child has obstructive sleep apnea (OSA), you may want to consult first with your child's primary care provider (usually a pediatrician or family physician) and share your concerns. You may also choose to consult with an otolaryngologist (ear, nose, and throat specialist or ENT) or a pulmonologist (a specialist in lung problems) who deals with children. ...
- Doctors who specialize in sleep medicine may also practice in your area. They have usually trained under other sleep specialists and/or studied sleep medicine through a residency program, continuing medical education (CME) courses, and scientific meetings. Some are certified by the American Board of Sleep Medicine (ABSM) as well, although there is not a separate certification for pediatric sleep specialists. You should feel free to ask any doctor or health care provider about his/her credentials and experience, especially in dealing with children. You should be satisfied with the explanations of what sleep apnea is and how it is diagnosed and treated in your child's particular case.
- In most cases, the initial evaluation for children with suspected OSA includes a complete medical history (symptoms; previous and current medical problems; operations, especially removal of the tonsils and/or adenoids; medications; and allergies), a review of any behavioral or developmental problems, a sleep history, and a physical exam (including weight and height). ...
- Based on the initial evaluation, your health care provider may suggest an overnight sleep study. A sleep study or polysomnogram can help to make a diagnosis of OSA in children and can help to judge the severity of the problem. ...
- The parts or components of a sleep study are very similar in adults and children. These generally include an electroencephalogram (EEG) to measure brain waves and an electroculogram (EOG) to measure eye and chin movement, both to monitor the different stages of sleep; an electrocardiogram (EKG) to measure heart rate and rhythm; chest bands to measure breathing movements; and additional monitors to sense oxygen and carbon dioxide levels in the blood as well as monitors to record leg movement. ... Still the process may be a little frightening for a young child; hence, most sleep labs accommodate a parent's stay with the child overnight. ...

- Other causes of sleep problems include anxiety, fear and worry. Some young children feel removed from their parents' love at night and their sleeplessness may be caused by separation anxiety.
